Vermont Chronic Care Initiative (VCCI)

Medicaid beneficiaries with one or more chronic health conditions benefit from the Vermont Chronic Care Initiative (VCCI). This program is free to those who are eligible through a partnership of the Office of Vermont Health Access (OVHA) and APS Healthcare.
 
Beneficiaries qualify for VCCI if they have one or more of the following chronic conditions: 
  • arthritis
  • asthma
  • chronic obstructive pulmonary disease (COPD)
  • chronic renal failure
  • congestive heart failure (CHF)
  • depression
  • diabetes
  • hyperlipidemia
  • hypertension
  • ischemic heart disease
  • low back pain.
 
VCCI focuses on the whole person.  Registered nurses and medical social workers encourage healthy behaviors and help with related issues such as housing, food security, and transportation to medical appointments. They help beneficiaries talk with their health care providers and may meet with both to develop and support a plan of care.
 
VCCI is a tiered approach. Services vary to meet each beneficiary’s needs. Changes in service tiers are seamless.
  • Beneficiaries with complex health problems receive face-to-face case management from a registered nurse or medical social worker to
    • coordinate care among health care providers
    • connect with other resources available in communities and from the state 
  • Beneficiaries at lower risk receive health education and coaching from registered nurses over the telephone 
  • All VCCI participants learn to better manage their own health conditions and to work with their health care providers.
