Consulting Services for Population Selection and Program Monitoring of Vermont Medicaid Chronic Care Management and Care Coordination Programs

The State of Vermont, Agency of Human Services (AHS) Office of Vermont Health Access (OVHA), is issuing this RFP to individuals and organizations that specialize in providing Medicaid population selection, stratification, and chronic care management (otherwise known as disease management or population health management) program monitoring services. The RFP describes the State’s newly-launched Care Coordination Programs and a proposed Chronic Care Management (CCM) Program for beneficiaries enrolled in its publicly funded health benefit programs, including Medicaid and the Vermont Health Access Plan. It solicits proposals from chronic care management consultants to provide scientific population selection and monitoring services.
