Request for Information for Care Management Services

The Office of Vermont Health Access (OVHA) is reaching out to organizations and businesses with expertise in providing care management services. OVHA is soliciting information from care management program vendors. The information which OVHA anticipates receiving will be used by the State in developing a Request for Proposal (RFP) for Care Management Services in the second half of calendar year 2006. The preliminary components of the scope of work include: services integration and collaboration; care management for general care and higher intensive care; and measurement of results for the overall success of the program.
